The goal of this meeting will be increase visibility and communication around projects that bridge the clinical-community divide as we work together to define and implement "Population Health" in Western Mass.  

Primary audience: anybody actively working on projects that benefit the health of the Western Mass population and involve some level of coordinator or collaboration with healthcare providers.
